New Tax Client Information Form
This form will tell us information about you and your household. Before we can take you in as a client, you will need to fill out this basic information, either online, or by coming to the office and filling out the form there. Please note that this applies only to people who need to file an individual / self-employment tax return. Business owners, trusts, estates, and non-profits should visit our Contact Us page, and ask for special instructions.
To begin, we will need to know if you are starting a brand new "New Client Form", or if you are continuing a form that you have already started.
Please note that if you do not complete the form within one week of starting it, you will have to start over.
